Dorset Help and Kindness are helping patients get home quickly and safely after a stay in hospital.
They work with patients, their families, the care coordinators, and discharge teams to see if there are community services to support. This includes working closely with the Volunteer Centre Dorset and other community organisations to provide immediate, intermediate, and longer term for support for people when they get home.
They link to services that can make sure there is food in the fridge when people come home, and can set up welfare checks, or help with getting prescriptions, or looking after pets. They can source free services to help with adaptations and practical support at home; doing the garden, replacing light bulbs, fixing handrails, or moving beds. They have access to a wide range of care services in the private sector which can be called on when needed.
